FFI believes that the elder population is at risk for their physical well-being during the COVID-19 crisis and their mental and emotional health due to the necessarily stringent social-distancing and quarantining guidelines now and for the foreseeable future. Because of this, it is essential to design programs right now to serve residents and those that care for them, be they family and/or staff. Your participation in this survey will ensure that as many perspectives as possible are incorporated into the design, delivery, and outcomes of these programs.

Friends Forever International (FFI), with support from Tufts Health Plan Foundation, developed this survey to identify and measure older populations' social interaction by receiving feedback from staff, residents, and family members from a range of older adult living facilities.

Our goal is to use this data to co-design, with older adult facility staff, their residents, and the resident's family members, a curriculum to integrate youth and older people using FFI's 30 plus years of innovative youth work and curriculum creation.

Why have I been sent this form?

You have been invited to complete this survey because you fall within one of the three categories (staff, resident, or family member). Your participation in this survey is voluntary; you can choose not to participate. If you start the survey and decide that you no longer want to participate, you can stop answering the questions at any time to withdraw from the survey. Your response will only be considered if you complete and submit the survey.

Is the survey confidential?

Your individual responses will be confidential. FFI will not collect names of yourself or the facility, email addresses for elder and youth integration, nor IP addresses - unless you specifically ask to be sent the results of the survey and/or receive communication about FFI programs. The survey does not ask about any confidential or medical information. Please be aware that the analysis of the survey results will be posted on a public Google Site. If you refer to yourself or your facility in your survey response, FFI will redact responses prior to publishing results to protect participant confidentiality.
